Gum inflammation (gingivitis) as a rule are responses to bacterial dental plaque. This may be removed with a thorough dental care or professional oral hygiene. For inflammations due to bacterial dental plaque applies: brush, brush, brush! Even it hurts a bit and will bleed.

First, clean the interdental spaces with floss, then for at least two minutes thoroughly cleaning with a (electrical) toothbrush. Above all, also the less well reachable areas of the rear teeth! Finally it is recommended to rinse with a chlorhexidine-containing mouth rinse solution (e.g. Curaprox®, Chlorhexamed®). Also small injuries – like, for instance, caused by chewing or hard bread and pizza crusts or the use of toothpicks – will result to local inflammations. Then you should treat the injured areas with care for some days and brush very carefully.

Normally the most gum inflammations recede within a few days. If this will not be the case and the gums will bleed for more than one to oneandahalf weeks, please see your dentist. Untreated gingivitis easily will lead to an inflammation of the periodontium (periodontitis) and eventually will be followed by a tooth loss.

There are numerous causes of bad breath, however, it is definite that it develops in the oral cavity. Not only food residues that have deposited between the teeth and decompose cause bad breath but also leaking fillings and crown edges, or inflammations like in the gingival pockets may be responsible for it.

In the most cases it is caused by bacteria that settle on the back of the tongue, especially in the rear area. Water picks, chewing gum, and breath sprays suppress bad breath for a short time but do not tackle the cause. Additionally to a proper oral hygiene it is recommended to use a special tongue daily to scrape the tongue. These are available in pharmacies or drugstores. A useful support may be mouthwashes with anti-bacterial solutions but you should not take them for more than two to three weeks.

According to scientific studies it is estimated, that about 30 percent of all people press or grind with their teeth while sleeping (bruxism). In the morning you may experience pain in the jaw joints, damaged dentures, muscle tenseness in the collar or neck, or headaches. The cause for grinding with the teeth could be stress or permanent mental strain. Likewise crooked teeth may trigger and maintain bruxism since this is a gliding obstacle for the chewing movement.

Bruxism is some sort of a stress outlet – while sleeping one is trying  to reduce stress. Therefore, in these cases a dental therapy will be imperative to protect the involved tissue, like jaw joint, articular cartilage, muscles, and teeth in good time. The so-called “bite splint”, that is similar to a tooth protector for boxer, may provide the teeth with an effective protection.

The cause for sensitive tooth necks is, that, as time goes by, the gums form back and the tooth necks will be exposed. Since the tooth neck does not have a protective enamel they are very sensitive. Also the elution of important sealing minerals of the tooth substance – e.g. due to sour and sweet food – may result to a sensitivity. Cold, heat, or chemical stimuli therefore will be experienced very strong.

The degeneration of the gums frequently is due to too fierce brushing. Other causes may be: bruxism, adverse tooth position, or a poor orthodontic care. The treatment may be provided, on the one hand, using special toothpastes or fluoride varnishes applied by the dentist. Unfortunately, the effectiveness is limited.

Bacterial tooth plaque provides the condition for caries. It is composed of bacteria and their metabolites and develops due to the decomposition of carbohydrates like sugar, sugar-containing products, flour products, e.g. pastries, potato chips etc.. Also time is an important factor because if we do not grant time for the bacteria to produce the caries supporting toxins no caries will occur. This refers to frequent brushing to reach all tooth areas. Consequently the difficult to access parts will develop caries at first and therefore such parts – the interdental spaces and all surfaces of the rear teeth – should be cleaned very properly.

If the plaque will not be removed in good time then toxins develop that will corrode and dissolve the dental enamel. At an early stage, caries may be stopped with an intensive oral hygiene and fluoridation. But if the caries already has penetrated the dentine than it will spread relatively fast towards the pulp. In this case only the dentist may help: by drilling and a filling.

Five factors contributing doe the development of caries – and what we can do to prevent them:

  • Plaque – mechanical removing
  • Bacteria – their concentration in the mouth will be reduced by plaque removing, mouth washes, and fluoridation. Additionally, a tongue toothbrush should be used to abrade the tongue. A ‘large oral hygiene at home’ should be performed once or twice a week, meaning brushing the teeth for 4-5 minutes supported by a water pick and a mouthwash.
  • Substrate – change of the nutritional habits: sweets only after the main meals
  • Time – don’t grant time for the bacteria to built-up toxins that damage the dental enamel. Therefore, brush your teeth at least three times a day!
  • Acid – the acid built up by the bacteria boosts the dissolving of the protective enamel and makes it more vulnerable. Brush regularly to avoid the development of acids.

It hugely matters to have the ‘hygiene ability at home’ reconstructed by regular professional oral hygiene sessions in our practice – at least two times a year. Because where tartar or other hard plaques exist you may care regularly and time consuming as you like but in the niches and covered areas caries may occur.

Tartar develops if plaque adheres to the tooth for a longer period of time. The dental plaque ingests mineral compounds of the saliva and gets hard – this will change to tartar.

Visible tartar grows rapidly but frequently may be removed by the dental hygienist with a scraper or ultrasound. In this case, the toothbrush alone may not help anymore. Tartar always will be coated with plaque and must be removed regularly. Therefore, two times a year a professional oral hygiene should be performed by a dentist.

Toothaches always have a cause that must be eliminated. To ignore the pain or bravely suffer them is no wise alternative. Frequently the first pain disappears again – but in the most cases it will come back, but unfortunately stronger.

A not or too late treated tooth will affect the pulp. This infection may spread to the jawbone via the openings of the apex and will stress the entire organism.
